June - RNCN Monthly Meeting "Interested in Industry Standards regarding the role, case load, and skill level and performance measure for the Case Manager?"
Stefany H. Almaden RN, MS, CCM, CPUM, CMCN


Stefany is a Care Management and a Life Care Planning Specialist, Consultant, Business owner with over 25 years of service in different settings within the Health Care Industry.

Most of Stefany’s professional experience involved working with vulnerable populations: Medicare/MediCaid beneficiaries, catastrophic cases of Spinal Cord Injuries (SCI) and Traumatic Brain Injuries (TBI), Stroke patients, and other life-altering events.

Stefany is also chairing the Care Management Institute of a National Nursing Association (AAMCN) setting industry guidelines for Care Management Specialty and is serving on the Board Directors of AAMCN.

Stefany is currently working on her Ph.D. in Health Services, and continues to advocate for access to care issues. Stephany will be discussing the following:

  • Role of Care Manager
  • Reasonable Case Load
  • Skill level necessary to perform Care Management Functions      
  • Performance measures for measuring the effectiveness of Case Management.
  • Defining Case Manager Role with the new CMS IPPS Final Rule
